FAQ – Frequently Asked Questions

Frequently Asked Questions

Fees & Insurance

Do you accept insurance?
+

We are an OUT-OF-NETWORK provider.

Luminous Vitality Behavioral Health does not accept insurance directly. We require full payment at the time of service.

For patients with PPO, EPO, or other insurance plans that include out-of-network mental health benefits, we provide detailed superbills that you can submit to your insurance company for potential reimbursement.

The amount of reimbursement depends on your specific plan’s out-of-network mental health benefits, deductible, and coinsurance rates. We recommend contacting your insurance provider to verify your out-of-network mental health coverage before your appointment.

What are your fees?
+
  • Initial Evaluation (75 minutes): $400
  • Follow-up Appointments (25 minutes): $250

Payment is due at the time of service. We accept all major credit cards and HSA/FSA cards.

We provide superbills for insurance reimbursement if you have out-of-network benefits.

How do superbills work?
+

After each appointment, we provide you with a detailed superbill containing all the information your insurance company needs to process reimbursement:

  • Provider information and NPI number
  • Diagnosis codes (ICD-10)
  • Procedure codes (CPT)
  • Date of service and amount paid

You submit this superbill to your insurance company, and they reimburse you directly according to your out-of-network benefits.

What if I need to cancel or miss my visit?
+
  • Cancellations made more than 48 hours in advance will not be charged
  • Cancellations between 24-48 hours in advance will be billed half the visit fee
  • Cancellations within 24 hours or missed appointments will be charged the full fee
  • Being more than 15 minutes late constitutes a missed visit or “no show”
  • Insurance does not reimburse for cancelled or missed appointments
